Rails run along the bed. A grab handle is one fixed point, positioned where the hand actually goes when someone sits up and swings their legs out.

For a person who is mobile and simply needs something solid to hold at that moment, a handle is often the better choice. It gives the support without enclosing the bed, avoids the entrapment considerations that come with longer rails, and looks far less like hospital equipment in a bedroom.
